How do I make themes?

I've read a few posts on this specific topic, but I still find it a bit confusing on how to make a custom theme. Is there a template I can download and edit from there?

 

I have tried making a theme and I struggle with some things like how to make the theme transparent or how to import an image onto the header.

Here's the psd template for custom images: D2ThemeHelper_0.2 (2).psd

Alternatively, here's a list of the names and sizes (in pixels) for each image:

header (350x75)
options (350x450)
breadcrumb (350x30) - this is the bar where it tells you the path like root/interface/menu-themes
color (275x385) - this is the color picker panel
info (350x555) - this is the online player info panel
input (350x77) - this is the input panel
scroller (10x100)
tips (350x30) - this is the bar on the bottom of the options list that gives tips & hotkeys

How to import images? 

- Each image must be placed in the theme's folder and must have an exact name as stated in the list above (check my folder's picture to understand)

- You're not forced to use each of the images, for example if you want to only have a custom made header image, only put you custom "header.png" file in the theme's folder

Here's an example of how one of my custom theme's folder looks like:

- If you're operating while the menu injected, in order to apply custom images, you have to go to Interface -> Menu Style Settings -> Reload Menu Images

- Transparency is given by the "Alpha" value in the color picker, that's the 4th value, setting it to 0 will give maximum transparency.

- Remember that when using custom images you'll have to play with both the Background colour transparency and the Custom Image (say for example, the header)'s transparency, you can find all the colors related settings in Interface -> Menu Color Settings
